    ENGINE

Component: Engine — Engine Cooling System — Coolant Pump Summary: The electric water pump on my 2019 Toyota RAV4 failed prematurely at 64273 miles. The check engine light illuminated with diagnostic trouble code P26CB (Engine Coolant Pump Performance/Stuck Off). The engine coolant temperature fluctuated rapidly / coolant level dropped. The vehicle required immediate repair to avoid engine damage. This is a recurring failure pattern on 2019–2021 Toyota RAV4 and Camry vehicles equipped with the 2.5L A25A-FKS engine, where the electric water pump seizes due to an apparent defect in its plastic housing and rotor assembly. Failures are routinely reported at 30,000–60,000 miles—far below the expected service life of the component. Loss of coolant circulation creates an overheating hazard and can lead to sudden loss of propulsion and engine damage while driving. Toyota has issued technical service bulletins related to coolant seepage at the pump but has not recalled the component or extended warranty coverage, leaving owners to bear repair costs of $600–$1,300 for a premature defect-driven failure. I request NHTSA to evaluate this defect for a safety recall.

As soon as my vehicle hit 100K mileages and there's "AWD System malfunction" sign showed up on dash. Got it done replaced it cost roughly more than $500. After 2 days later another problem shows up on dash this time it was "engine maintenance required" show on dash again. Heard people had the same issue to whoever own 2019 RAV4 and they all had the same for those 2 issues. Toyota company should pay attention for these kind of problems or like some of of recall.

FUEL/PROPULSION SYSTEM · FORWARD COLLISION AVOIDANCE

While driving in the mountains on a hot day, the car suddenly reported numerous faults, including check engine, parking brake malfunction, pre-collision system malfunction, AWD system malfunction, and secondary collision brake system malfunction. The car lost power and forced us to pull over on the side of a dangerous mountain highway. We powered-off the car for a few minutes and started it again and were able to proceed to our nearby remote mountain destination without another loss of power, but the faults remained. The next day we were able to get the car to a nearby NAPA store to read the check engine codes, which there were only 2 of the "P0087" code. I had them clear the fault codes in order to be able to safely drive the car (~120 miles) back home. Some quick research shows a known issue and TSB for that in our exact high-altitude and high-temperature situation - Toyota T-SB-0058-22, related to a known design flaw with the ECM and fuel suction tube. Luckily, it did not throw the errors again during our drive home. I made an appointment at our local AutoNation Toyota Arapahoe for the following week. I detailed the issue and provided pictures of the errors - including the code reader display. The dealer claimed to not find any issues and refused to address/execute the TSB, stating that the code must be active and present in order for them to fix it and that my pictures were not sufficient proof. I explained that it was an unacceptable safety risk to have left the car in that state and that the issue is known to only occur in certain conditions (e.g. high-altitude, high-temperature mountain driving). Issue remains un-addressed, which is highly unacceptable and poses a serious on-going safety risk.

    STEERING

We were driving down the highway passing another vehicle when we lost steering and the dash said "Steering Power Loss". We pulled to the side of the road where the vehicle stalled and would not restart. Probably 10-15 minutes later the vehicle started and we eventually made it to our destination. It was quite scary trying to get to the side of the road, in heavy traffic, and pulling over where there really was no shoulder, and not knowing whether the car would eventually restart or what. Although 2019 Toyota Rav4's have been recalled for this very issue, for some reason our VIN is not listed.

Display of RAV4 Hybrid is malfunctioning as it randomly turns on and off at frequent intervals, the screen turns on an off in a similar manner, the display controls are not operative even if displayed. This occurred right after the Dealer (355 Toyota of Rockville, [XXX] ) conducted a recall procedure on a wire harness. Immediately after these repairs, it was noticed that the display started to malfunction. It has never malfunctioned before. I went back to the Dealership and the manager said that the display problem was unrelated to the recall maintenance. So I am reporting it here. While driving down a county road at approx 40mph, the glass of the sunroof of my 2019 RAV4 suddenly cracked, with shards falling down into the vehicle. There was no apparent cause or impact. After moving to a safe location to further inspect, I noticed the following details; - the rectangular glass of the sunroof is cracked through at multiple points along the edges of the pane, and a "web" pattern of partially shattered glass extends across the whole pane. - there is no pattern that I would associate with an impact spot and no evidence of impact from any falling object. At the time, I was driving under open skies in a rural area, not under any trees, overpasses, etc. - the glass appears to be buckled outwards, as though internal cabin pressure was a factor - the car had been parked outside my office all day, the day was overcast with a high in the 80s. There was no rain/hail/etc. Overall, the lack of apparent impact and outward-buckling pattern raise concern for a material or manufacturing defect. While no one was hurt in this incident, a similar failure could have been a significant problem if it occurred on an interstate or other location the operator could not pull over, or during adverse weather. The vehicle is being taken for inspection at Stapp interstate Toyota, Frederick CO on 29 July 2026.

    STEERING

While driving the 2019 Toyota RAV4, I found that the steering wheel was getting hard to turn and the car kept making grinding sound when turning. Due to I was short on time, I asked an old mechanic friend to check the RAV4 out before my road trip. He was unable to get the vehicle back; therefore, I used my other vehicle. While I was out of town the Mechanic contacted me, and ask if the car had been in water and I replied no. He suggested that I might want to see if there’s a recall. I googled recall for RAV4 vehicles and found that a recall had been issued for certain groups of power, steering boxes. This may allow water to enter the box that could corrode and cause loss of power steering that could cause an accidents. I’ve reached out to several Toyota representatives with this issue, but have been unsuccessful and getting them to see that this car may have been overlooked.

The contact owns a 2019 Toyota Rav4. The contact stated that while driving at an undisclosed speed, an orange steering symbol appeared on the instrument panel accompanied by the message: "Power Steering Malfunction, Steering Power Low, Visit Your Dealer." An initial inspection by an independent mechanic indicated that the electrical power steering and the rack and pinion assembly needed to be replaced. Subsequently, the vehicle was taken to a dealer, where diagnostic testing confirmed a defective power steering system. Following a software update performed by the dealer, the warning symbol transitioned to red, accompanied by an audible alarm and a significant increase in the physical force required to steer the vehicle. Restarting the vehicle temporarily reverted the warning light to orange and restored steering functionality. Upon investigation, the contact discovered NHTSA Campaign Number: 20V373000 (Steering); however, the VIN was not included in the recall. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline to report the failure. The vehicle was not repaired. The failure mileage was 105,857.

Rav4 2019 - We lost engine power while driving on the highway at 75 mph. We were in the #2 lane and were fortunate to be able to coast to the side of the road. Quite scary as there were semi trucks behind us.We had been driving for approximately 3.5 hours on the highway at 70-75 MPH. The outside temperature registering on our vehicle began at 110 and continued to increase on up to 118 degrees at the time of the incident. When we lost engine power, the following errors began flashing on the dashboard: PKSB Malfunction, Pre Collision System Malfunction, Secondary Collision Brake System Malfunction. We cleaned the sensors, turned the car off for 5 minutes and then turned it back on. Same warnings appeared. Got towed to the Toyota dealership. Next day all malfunction warnings gone. Car was behaving normally. Toyota service team was unable to reproduce the problem. They inspected the electrical, battery, sensors, etc. but could not find anything. Ultimately the service lead stated that the intense heat could have caused sensor malfunction. Scary!! We fortunate!

The 8-speed automatic on my 2019 RAV4 (2.5L 4 Cyl) has been shifting super rough at lower speeds, to the point to where downshifts can send the revs into redline territory, but the car really doesn't move. No warning lights, passed state inspection, recently serviced. purchaced this car in Janurary with 50k miles on it and noticed it within a few weeks of driving. I am now at 57k and the issue is signifigantly worse than it was before. Will be taking it to Toyota soon. The safety issues arise when I do not know how the car is going to accelerate or respond to throttle input in traffic. I don't trust the car to handle any fast throttle input if I need to move out of the way quickly.

The rear door (lift gate) hinges are failing and tearing away from the vehicle body, causing misalignment of the rear door, operational failure of the automated lift gate, and water leaking into the interior. This has occurred under normal use conditions. It appears that the hinge design is insufficient to handle the load and forces of normal operation of the door. The failure has been documented in pictures. The Toyota dealership where the vehicle was purchased stated that the vehicle is out of warranty and there is no recall for the hinges, therefore the owner is responsible for repair costs. Had the issue not been found and the hinges failed completely, it could have been extremely hazardous.

My 2019 Toyota RAV4 Adventure suffered a sudden catastrophic transmission failure while driving on the highway on April 28, 2026, resulting in loss of forward gears and a dangerous situation. On April 28, 2026, while traveling in [XXX] , my 2019 Toyota RAV4 Adventure (159,173 miles) experienced a sudden transmission failure with no prior warning symptoms. The vehicle began slipping, made a whining noise, would not properly upshift or hold speed (became stuck in 3rd gear), and eventually lost all forward propulsion, creating a safety hazard on the road. I had the vehicle towed to Zanesville Toyota in [XXX] . Their diagnosis (copy of repair order attached) revealed severe internal damage including: Torque converter clutch actuator stuck off Multiple pressure control solenoids stuck Transmission slipping and complete loss of all forward gears Fluid full but contaminated with metal debris and clutch material Whining noise The dealer quoted approximately $15,000 for a full transmission + torque converter replacement and denied goodwill assistance. This appears to be part of a known issue with the 8-speed automatic transmission (UB80) in 2019+ Toyota RAV4 models. I am concerned this is a safety defect that can cause sudden loss of power while driving, increasing the risk of a crash. I have attached the Zanesville Toyota repair order dated 04/30/2026 documenting the failure. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I am reporting a serious concern and safety issue regarding the SlimFit3 LX car seat. We purchased it for a [XXX] old [XXX] male. After adjusting the head rest to the proper height so that the harness straps were slightly below his shoulder, we placed him in the seat and his head was unnaturally pushed forward and down so that his chin was pushed into his chest. This was caused by the headrest sitting too far forward of the back padding so there was no way for him to keep his head in natural alignment with his back. The contributing factor is the minimal back padding and its inset into the car seat. There should be more padding to move the back forward or the headrest should be retracted into the seat more. We immediately returned the two car seats we purchased because of this and went with a different manufacturer. Not only is this an ergonomic/comfort issue, but it could introduce airway obstructing breathing issues if the child fell asleep. It is an extremely unnatural position. Also, I would be concerned about long term damage to a child's posture after repeated use. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

A coolant leak from the flow shut‑off valve caused the engine maintenance warning light to remain on for an extended period. This issue is covered under Toyota Customer Support Program 24TE04. The warning light is a safety concern because it indicates an active engine‑related malfunction and can mask other warnings. The defect was first diagnosed by a Toyota dealership at approximately 97,000 miles, well within the CSP coverage window. The dealership stated the vehicle “did not meet criteria” and refused to perform the repair, even though the symptoms matched those described in the CSP. The issue continued until a second Toyota dealership later confirmed the same defect and replaced the coolant bypass valve. Because the vehicle had since exceeded 100,000 miles, Toyota refused to cover the repair under the CSP, despite the defect being documented earlier within the coverage period. Multiple attempts to resolve the issue with Toyota corporate have resulted in no returned calls or meaningful response. The defect caused prolonged operation with an illuminated engine warning light, which presents a safety risk and indicates a failure to properly address a known issue covered under Toyota’s Customer Support Program

Instead the car kept accelerating down an embankment, across an empty street and towards a telephone pole. Turning the steering wheel to the left to avoid the telephone pole the car further descended into a ditch. The car had to be towed and is now deemed to be totaled. The brake- pads had been inspected during a routine maintenance check the day before this accident occurred and they were good. No other vehicle was involved. My personal injury was an abrasion and bruise where my seat belt did it's job. My muscles are sore. The side airbags did deploy after the car stopped. The floor mats were locked in place.

Safety Defect: Data Communication Module (DCM) Failure Affecting Hands-Free Microphone and Safety Connect / SOS Emergency System Vehicle: 2019 Toyota RAV4 Hybrid LE The DCM (which controls both the CarPlay/Bluetooth hands-free microphone and the Safety Connect SOS emergency button) suddenly failed without any warning or accident. Symptoms: - No one can hear me during CarPlay or Bluetooth calls (microphone inoperative). - Safety Connect / SOS light became intermittent or inoperative (directly impacts emergency calling, crash notification, and roadside assistance). This is a known recurring issue documented in Toyota TSBs T-SB-0112-20 and T-SB-0089-21. Toyota of Hackensack diagnosed it and quoted $1,300 for DCM replacement/update while charging me $200 for diagnosis. They did not disclose during the multipoint inspection that this is a documented safety-related DCM fault. I performed the manufacturer-recommended 12V battery disconnect reset (standard TSB procedure), which fully restored the microphone and returned the SOS light to solid green. However, this is only a temporary fix for a known defect. This failure compromises hands-free calling while driving and the emergency SOS system — both critical safety features. I request NHTSA investigation and urge Toyota to issue a recall or service campaign for all affected 2019–2025 RAV4 Hybrids.

I have noticed visible rust forming on the roof, specifically the front drivers side adjacent to the inside edge of the roof rail that runs forward to aft on the vehicle. The same stated rust spot is near where the roof meets the windshield. It is available for inspection upon request. My 2019 Toyota Rav4 along with several other 5th generation Rav4 owners online are experiencing rust in the same exact spot. This rust may be a result of the Toyota Customer Service Program (CSP) #22TE05 and T-SB-0016-23 that came of a result of the Fishkind v. Toyota Motor Sales, USA et al. No. Case 2:23-cv-02279 Class Action Lawsuit. This rust may have been formed by the leaking roof rail gaskets that have a slow enough leak rate to penetrate the roof rail clip holes and create moisture and rust over an extended period of time but not high enough of a leak rate to cause excess leakage into the vehicle that would immediately alarm the driver to have the vehicle taken into the dealership for immediate repair before the corrosion and rust can have a chance to spread. After several conversations with Toyota Corporate and my Toyota dealership they have denied me CSP inspection and any coverage or good will assistance through the CSP# 22TE05 & T-SB-0016-23 despite evidence of water residue buildup on the headliner visible through a UV blacklight. Even if the rust is not caused from the roof rail gasket leakage, there are still multiple other instances online of other 5th gen rav4 owners experiencing roof rust in the same exact spot adjacent to the drivers side roof rail near the windshield, which points to a manufacturing defect. My safety will be put at risk when the corrosion worsens and leaks through into the electrical components of the vehicle. My dealership witnessed the rust but denied performing an inspection of the roof rail clips since there is currently visible rust, likely due to the roof rail clips leakage. No manufacturer inspection. No noticeable warning symptoms.

The rear liftgate began malfunctioning and would intermittently fail to open or close properly. The issue had previously been reported to a Toyota dealership, but the problem could not be reproduced at the time. I noticed recently that the liftgate was not fully closing on the passenger side. While inspecting the liftgate more closely, I discovered that the passenger side liftgate hinge is cracked and the metal at the hinge base is separating. The crack appears to run through the hinge bracket where it attaches to the body. The driver side hinge also shows signs of stress. Because the hinge is cracked and separating, there is concern that the liftgate could eventually detach or fall unexpectedly. The liftgate is heavy, and a hinge failure could allow it to drop suddenly, posing a risk of injury to someone standing near or underneath it. If both hinges fail at the same time, the liftgate could become detached from the vehicle while driving resulting in an accident and potential injury to myself, my family, and others that are hit by or hit the liftgate. The dealership suggested the damage may have occurred from forcing the liftgate closed when the liftgate struts failed to function. However, the cracking appears consistent with progressive metal fatigue rather than a single impact event. This appears to be a structural hinge failure that could affect the safe operation of the liftgate. The dealership has documented my concerns on numerous occasions and today they visually saw the hinges in their current shape.

On February 2, 2026, while driving approximately 65 mph on the freeway in Wisconsin, the panoramic sunroof glass on my 2019 Toyota RAV4 XLE Premium (VIN: [XXX] ) spontaneously exploded. The ambient temperature was approximately 20°F. Crucially, the glass buckled outward, indicating the failure was caused by internal pressure or a structural defect, rather than an external impact (such as a rock). There were no other vehicles immediately ahead of me that could have kicked up debris. Safety Risk: The explosion created a loud, gunshot-like noise that caused severe driver distraction at highway speeds. While no crash occurred and no injuries were sustained this time, the sudden structural failure posed a significant risk of loss of vehicle control. Status: No warning lights or messages appeared prior to the failure. The vehicle had no prior damage to the roof area. The hinge has broken from the body, causing the liftgate to fail to close the back trunk area of the vehicle. The defective hinge has caused misalignment of the door and an unsecure compartment which is a violation of NC State Law according to NC GS 20-116. This puts the passenger's and driver of the vehicle and other surrounding vehicles at risk by failing to stay aligned or seal properly. I fear that glass may crack due to the currently misalignment. I have sought out further information from a Vehicle Repair Center, who confirmed there is a current Lawsuit on the Case No.: 5:24-cv-00912ZACH HUGHES, individually and on behalf of all others similarly situated, Plaintiffs, v. I was driving down a road and my engine just stopped working lights came on saying there were several issues including the engine. I researched online and found there were recalls due to this same problem for the same make, model and year as my Vehicle, but there is no recall on my vehicle. The dealer confirmed the engine was seized up or whatever wording they used. No one has inspected it other than Rodland Toyota. There were no warning lights or messages or symptoms before the issue.

The rear trunk door would occasionally fail to open or shut for the last several years. It was inconsistent with this behavior and I was never able to get it to fail when at the dealership servicing department. This weekend when at the store, it failed shutting and I had to force it to shut. While driving home it rattled itself open. I had to pull over to inspect it and could not get it to close. When I looked closer, the hinge of the door had been ripped from the body on the passenger side of the vehicle. I have never been in an accident nor have I ever had any impact on this door.

Infotainment Center turns on and off, and will not work. Because the screen turns on and off, the camera for back up intermittently works. May 2025 i took it to Karl Malone Toyota in Sandy Utah. They said to buy a 3rd party, after investigation I confirmed that no display has the options to reset maintenance, change clock on the dashboard, and communicate with the car. Yes radio, navigation, carplay, etc work with a 3rd party system, but the functions that talk with the car do not work on any 3rd party system.

Vehicle consistently accepts only approximately 9–10 gallons of fuel when the fuel gauge indicates empty, resulting in significantly reduced driving range. Issue has been widely reported by other Toyota RAV4 Hybrid owners and appears related to fuel tank design. Manufacturer declined assistance and referred responsibility to selling dealer despite known complaints.

I began experiencing intermittent rear liftgate sensor issues on my 2019 Toyota RAV4 in late 2020. The power hatch would stop halfway while opening. On 3/24/2021, while the vehicle was under factory warranty, I brought it to a Toyota dealer for inspection. The dealer stated the issue could not be replicated and advised me to manually push the hatch upward. No repair was made. Over time, the condition worsened. The liftgate began intermittently stopping halfway while closing, using both the key fob and the interior dash button. In 2022, a rattling noise developed in the rear of the vehicle, later determined to be associated with progressive liftgate structural failure. By 2024, the rear liftgate became unsafe and largely inoperable. The hatch shifted significantly to the left, leaving an abnormally large gap at the top and driver’s side. I stopped using the liftgate due to concern it could fail unexpectedly or not close properly. I contacted Toyota Engagement Services multiple times, explaining that this issue is widely reported among 2019 RAV4 owners and presents a safety concern. On 11/18/2025, the vehicle was inspected at Crabtree Toyota. The inspection confirmed rear liftgate hinge failure. The dealer provided video documentation and a written repair estimate. The health inspection also noted a separated and broken passenger-side rear hatch pulse sensor requiring replacement. Toyota closed my case without repairing the defect. Toyota offered a “good faith” credit of approximately 75% of the repair cost that cannot be applied to the liftgate structural or sensor repair and is only usable for unrelated future service. This appears to be a known, progressive defect involving rear liftgate hinges and related sensors on 2019 Toyota RAV4 vehicles. The condition presents a safety risk, as the liftgate may fail to stay open, close unexpectedly, or malfunction electronically. My VIN has been provided. Safety issue: The vehicle experiences intermittent loss of acceleration and transmission “slip/rev” events, especially when cold, from a stop, or at low speed. At times, the vehicle revs as if in neutral, then re-engages. The driver sometimes must press the accelerator fully to gain speed. In one instance, the vehicle was driven slowly for several miles, pulled over, shut off, restarted, and the function temporarily improved. When it started: Symptoms began immediately after a routine service visit at a Toyota dealership ([XXX]). First noted deceleration going uphill with recovery. Next day, multiple warning indicators/messages appeared and vehicle was towed to the same dealership. Dealer response: Dealership advised transmission replacement based on diagnostic codes and quoted approximately $10,000, stating fluid check was not necessary. Second opinion: Independent shop found transmission fluid level extremely low (reported approx. 1/4 full). They refilled fluid; initially vehicle drove normally, but intermittent slipping/revving continued after pickup (worse when cold/low speed). No external leaks observed by second shop at that time. Current status: Vehicle continues to show intermittent loss of acceleration/slipping behavior, creating a safety risk in traffic. No current check engine light, but symptoms persist. Codes/records available: Codes: P07457F, P28167F, P088A7F, P27137F, C137BA2, C1241, C1280, U1117, B2227. Location: New York. While traveling back from out of state, the fuel sensor and fuel gauge failed causing me to run out of gas when the fuel gauge stated I had just a tick under a 1/4 tank of gas left and the app read 35 miles left. While driving on freeway at freeway speed, the car sputtered, bogged down and then stalled, placing me in a very dangerous situation broken down on the shoulder of the freeway in the desolate country at night with semi trucks and cars flying by at estimated between 70-80 mph. While stalled and waiting for help, all the lights except the hazard flashers went out. The Sheriff assisted and added 2 gallons of gas that finally got the car restarted and I was able to get to the gas station which was 1 mile ahead of where I had been stranded. I kept filling the gas tank not allowing it to go under 1/2 tank on the fuel gauge the remainder of the trip home. I took the car to Tansky Sawmill Toyota for service, who thought it was a fuel sensor in fuel tank, which had to be dropped, resulting in a tube from the fuel sender being disconnected and not taking fuel from the other side of the tank and corrosion was cleaned out. They had to go through electrical system to check the fuel gauge which had corrosion on the electrical harnesses and sensors not caused by water or rodents - still unknown cause - resulting in $6500.00 to replace both electrical harnesses 82152-0R370 wire FR Door LH and 82141-oRB80 wire Instrument Panel! Known Toyota defect as the 2019 Rav4 Hybrids have already been recalled. Issues of this type with the regular Rav4s are posted all over the online Toyota boards. My platinum extended warranty on my well cared for 2019 RAV4 with 45,131 miles (purchased at 3 year lease end) refused to all of the charges and the dealership did not budge, sticking me with 6 hours of diagnostic fees over what the warranty allowed in the sum of $1014.37. These harnesses need recalled so no one else is stranded in a very dangerous place on the side of the freeway!

    SEATS

I am reporting a concern regarding the driver’s seat design in my Toyota RAV4. While driving this vehicle, I experience consistent back pain that can begin within approximately 15 minutes of driving and often persists for about two hours after driving has ended. The pain includes sharp discomfort in my mid-to-upper back and occurs despite adjusting the seat position and using aftermarket lumbar and seat supports. I do not experience similar pain when driving other vehicles for comparable durations, including a Toyota Highlander, Toyota Camry, Subaru Impreza, Ford Focus, Rivian truck, or a pickup truck. This strongly suggests the issue is related to the RAV4’s seat ergonomics rather than a preexisting condition. Because of this issue, I have begun avoiding driving my own vehicle when possible. I am submitting this report to document a potential seat design issue that may affect driver comfort and safety.
