    FUEL/PROPULSION SYSTEM

    While driving on the highway, the vehicle suffered a sudden loss of engine power and went into a low-power/vapor lock state due to severe fuel starvation. This created a major safety hazard by stranding the vehicle on the highway shoulder and exposing occupants to high-speed traffic. Prior to the power loss, there were no advance warning lights until the engine stalled. A diagnostic scan performed immediately following the incident confirmed code P0087 (Fuel Rail/System Pressure - Too Low), along with cylinder misfire codes P030027 and P030100. To allow the vehicle to safely limp home from a remote location, an independent technician cleared the diagnostic trouble codes. An independent scan tool report clearly documented and confirmed the low fuel pressure fault prior to code clearance. The dealership subsequently declined to perform a warranty or recall fuel pump replacement citing the absence of active codes upon arrival, despite receiving the timestamped diagnostic report of the failure. The component has not been inspected by the manufacturer beyond the dealership's initial turn-away, but the vehicle and diagnostic documentation remain available for inspection upon request.

    The rear liftgate is no longer aligned and does not sit evenly when closed. After noticing the uneven fit, I inspected the liftgate hinges and found that both metal hinge brackets appear to be cracking and splitting at their mounting points. There were no warning lights or messages before the problem appeared. I am concerned that the hinges could fail completely, causing the liftgate to fall unexpectedly, detach from the vehicle, or create a road hazard while driving.

    POWER TRAIN

    During summer 2026 i began to notice a noise/whine above 30mph that would only occur when foot is on accelerator. my vehicle has 83k miles on it (me being only owner with routine maintenance). as a responsible owner, i had my dealer do an inspection. after a drive and a listen, they claimed it was a transmission bearing failure and only solution is a 11k transmission replacement, with the commentary "we had a camry in last week with same issue". Being responsible owner, i got a second opinion from an independent shop who also verified "transmission bearing failure, you will need a new transmission. currently the part is on backorder because so many are needing to be replaced and no one should do a replacement with a used transmission". This is not the quality i expected out of toyota -- transmissions should not be needing to be replaced at 83k miles.

    Vehicle was parked in my garage and it was 80° outside. I was standing at the door and heard an explosion. I found out that the rear glass on my RAV4 SUV exploded.     **VEHICLE:** 2020 Toyota RAV4 LE **VIN:** [XXX] **CURRENT MILEAGE:** 126,776 miles **PURCHASE DATE:** October 2020 **COMPONENT:** Automatic Transmission (Power Train) **PROBLEM DESCRIPTION:** On XXX, I took my 2020 Toyota RAV4 LE to a transmission specialty shop (TJ'S Transmission Service Inc., Bedford, PA) because the automatic transmission was slipping while driving. The professional diagnosis (Invoice #XXX) confirmed the following stored fault codes: P2681, P2816, P08BA, P2713 The technician also confirmed that the transmission fluid was black and contained metal debris, indicating significant internal wear of clutch/band components. While researching these codes, I found that Toyota previously issued Technical Service Bulletin (TSB) T-SB-0160-18, titled "Transaxle Whine Noise, Harsh Shift, MIL ON, or Reduced Power," which acknowledges these SAME fault codes (P2816, P08BA, P2713, among others) as a known problem in the 8-speed automatic transmission (UA80/UB80 family, manufactured by Aisin). This TSB was officially issued only for the 2017-2018 Highlander and Sienna models, but the 2019 and later RAV4 uses the same transmission family (UB80E/UB80F) for the 2.5L engine, suggesting this is the same design or manufacturing defect that has not been officially acknowledged for this model. This issue represents a safety risk, as unexpected loss of power or transmission slippage while driving (for example, when merging onto a highway or passing) can create a dangerous situation for the driver and other vehicles on the road. I am requesting that NHTSA investigate whether a similar failure pattern exists in other 2019-2021 RAV4 vehicles equipped with this transmission, and that consideration be given to whether Toyota should extend coverage of TSB T-SB-0160-18 (or issue a specific one) to include the RAV4.     I ordered a plastic underbody cover (about 12×20 inches) through Toyota Arapahoe and paid $70 for the part. I was given an appointment for installation. When I arrived, I was told that the installation would take an hour and a half, even though the work consisted of fastening nine screws, while the old cover had been factory‑attached with only five simple self‑tapping fasteners, and this work was supposed to be free. After about an hour, I found my vehicle in the general parking lot, with no notification, no status update, and no explanation from the employees. When I asked why the work had not been completed, I was told: “There is not a single screw in the station,” “The screws may arrive in a week,” “The installation will cost about $100.” I asked the manager: “You want one hundred dollars just to fasten nine screws?” At that moment, the situation escalated. A man appeared who claimed to be “the boss of the entire station.” His behavior was aggressive and unprofessional. He refused to give his name or provide a business card. Because I speak English poorly, I asked him to say his name again. I tried to say “last name,” but due to stress I mistakenly said “famaly.” Instead of offering an electronic translator, this person began acting in a disturbing and irrational manner: shouting “family, family!”, rolling his eyes, moving around the room in an agitated way, and repeating the word loudly. I was confused and repeated the word after him, not understanding what was happening. Two other employees approached and began repeating “Go, go,” pressuring me to leave. Their behavior was clearly aimed at intimidation and forcing me out of the premises. After returning home, I measured my blood pressure: 170/90, which is significantly higher than my usual level of 140/80 and is considered a pre‑heart‑attack indicator. This dangerous spike in blood pressure was a direct result of the aggressive and humiliating treatment by the employees of Toyota Arapahoe.

    The contact owned a 2020 Toyota Rav4. The contact stated that while pulling into a vertical spot and making a right turn, the vehicle suddenly accelerated, jumped the curb, the sidewalk, and continued driving down the sidewalk and crashed into a building. The brake pedal was depressed but was stuck. The vehicle failed to turn off, and the parking brake failed to engage. There were no warning lights illuminated. The air bags deployed as needed. A Police report was filed. The EMS workers were unable to depress the brake pedal. The undercarriage of the dashboard ripped the skin off both shins. The contact was transported to the trauma hospital, where medical attention was provided for four weeks to close the wounds. A skin graft was performed on both legs. The vehicle was towed to a tow lot. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The vehicle was deemed a total loss by the insurance company. The black box data was retrieved, which showed the acceleration, but no information if the brake pedal was depressed. The approximate failure mileage was 25,000.

The contact stated about a gallon of water leaked from the rear passenger's side door panel. The vehicle was taken to the dealer, where the contact was informed that the repair was not covered under a recall. The vehicle was not repaired. The contact referenced an unknown recall; however, the VIN was not associated with the recall. The vehicle was not diagnosed or repaired by an independent mechanic or dealer.     If I don't drive the car for 1 to 2 weeks, the car's 12v is completely drained of power and the care needs to be jumped, otherwise, it will not start. This is consistent. I've taken this to several Toyota dealerships and they all have reported my 12v battery is healthy. However, Toyota is aware and is charging for the fix, labelled as T-SB-0089-21. The known issue is a piece of electronics in the car that continually drains power because it does not properly shut off. This should not be a paid fix. This should be a recall. When the car is jumped and the car finally starts, it reports a message saying something about some safety features not available. The car needs to be moved every 2 weeks due to mandatory city street cleaning so at least once every month the car needs to be jumped if not used the week prior.

The contact's father was a mechanic and was able to repair the vehicle. It was later discovered that Carfax reported the vehicle mileage as 176,009.

    Toyota made a defective coolant valve in their 2019-2021 cars that leaked spontaneously in these vehicles. Over 950,000 cars have been affected to date, including my 2020 Rav4. They are aware of the issue and made a customer support program for this issue which led to engines overheating/electrical code problems, which can cause a severe safety issue if left unrepaired. The program however has a limitation until November 2025 or 100,000 miles whichever comes first. When I was alerted of this, I brought my car to the dealer for service, but they wouldn't replace the part because it wasn't leaking too much at the time to need repair. However, just 4 months after the primary coverage expired, my engine code went off and when I brought my car, the coolant valve turned out to be defective and leaked into the electrical wire harness which then also had to be replaced in addition to the coolant valve. The repair cost was $1200 and both were required to keep the car driving safely and reliably on the road. Because my car had 120,000 miles and the known issue presented after Nov2025, Toyota refuses to cover the service, although it is a known manufacturing defect. Toyota is committing fraud to avoid covering this safety issue. The repair is expensive and if left unrepaired, the cars can spontaneously catch on fire on highways and in parks leading to wild fires and endanger others on the road. The toyota technician specifically informed me the issue was similar to other cars and known and the defect led to the destruction of the wire harness. To avoid a safety recall, Toyota reached a private settlement against class action lawsuit and created a limited CSP to avoid covering the defect in cars like mine to maintain their reliability status in consumer reports so cars like mine with little higher mileage won't be reported. It is fraud. I believe they are skipping on consumer repair bills that consumers on entitled to for their manufacturing defect.

    I am having a failure of my liftgate. My liftgate hinges are cracked and pulling away. The welds are cracked and weak. The liftgate does not close properly. The contact stated that while driving at an undisclosed speed, the brake pedal became inoperable and nearly traveled to the floorboard while attempting to stop the vehicle. A “Pull Over” warning message was displayed. The vehicle was towed to an independent mechanic, where it was diagnosed with diagnostic trouble code C1391. The contact was informed that the brake booster and master cylinder had failed and needed to be replaced. The vehicle was not repaired. The vehicle was then towed to a dealer, where the same diagnosis was confirmed. The contact was again informed that the brake booster and master cylinder had failed and required replacement. The vehicle was not repaired. The manufacturer was made aware of the failure and referred the contact to the NHTSA hotline for further assistance. The failure mileage was approximately 106,000.

This is now a safety concern as I’m afraid the other hinge is becoming overloaded and the entire thing could separate off from the car. This seems to be a common issue with my model and year. No warning of this. All of a sudden I could not close my trunk.

    Sudden and complete failure of the water pump causing an explosion of the part. While traveling down the interstate, put lives in danger due to sudden jolt and rapid deceleration. Recently brought the vehicle in to correct recall on the engine coolant bypass valve. Issue with the bypass valve hastened the deterioration of the water pump to the point of failure. Told by mechanic that this is a known issue that just happens with Toyota’s. If this is a known issue, then it seems to me there should be another recall on the water pump. Plastic parts with highly corrosive liquids at high temps do not mix. Toyota needs to fix this costly issue.

    2020 Hybrid RAV4 rear liftgate failure. The vehicle has been in no accidents and has never been damaged. Initially, it was a slight misalignment in 2022 and would still latch once opened and closed, but over time, the liftgate itself became separated from the vehicle body and the hinges are visibly bent. Initially presumed the sensor was being blocked somehow as it wouldn’t close as it lowered, but over time you could visibly see the alignment was off from the vehicle body. Now the tail lights no longer line up and there is large gapping along the top of the liftgate where it should meet the body of the car. Bent hinges were confirmed at a Toyota dealership and then confirmed at a body shop as well. The body shop informed me that they have seen and repaired this exact issue multiple times and recommended checking with the service center in case of any recalls or TBs. As of 2026 before repairs, the trunk is completely unusable as it has to be forced closed after use which threatens to compromise the structure of the vehicle. If the trunk cannot be latched or opened, it poses a safety hazard as a blocked means of emergency egress as well as a safety hazard of opening on the road if no longer able to latch.

    I am submitting this complaint regarding a structural failure of the rear liftgate hinges on my 2020 Toyota RAV4 XLE. The liftgate began opening and closing crooked, and I noticed the door was no longer aligned evenly. After inspection by Toyota dealership, I was informed that the metal hinges attached to the liftgate and body are cracked and must be replaced. This failure is not the result of an accident, misuse, or impact. I have never been in a collision involving the rear of the vehicle. I was told this damage is considered “wear and tear,” so my insurance company declined coverage. The Toyota dealership also declined to cover the repair, stating it is not covered under warranty so I will have to pay out of pocket. However, after researching this issue, I found that many other Toyota RAV4 owners have reported the same hinge cracking and liftgate misalignment problem. This appears to be a widespread manufacturing or design defect rather than normal wear. This is a serious safety concern. If the hinges continue to crack, the liftgate could fail completely and fall, potentially causing injury. The misalignment also prevents the liftgate from opening and closing properly, which affects vehicle safety and usability. I am requesting that NHTSA investigate this issue and consider whether a safety recall or technical service bulletin is necessary to protect drivers and passengers.     I am writing to formally report a significant structural failure regarding the power liftgate system on my 2020 Toyota RAV4 Hybrid XLE. Recently, the trunk began failing to close properly. Upon a detailed inspection, I discovered that the sheet metal surrounding the hinges has suffered catastrophic failure. The left-side hinge mounting point is severely fractured, and the right side is exhibiting clear stress cracks. This is not a case of "wear and tear." This is a structural failure where the force exerted by the factory power struts appears to have overpowered the vehicle's body structure, leading to metal fatigue and tearing. I would like to emphasize the following points regarding this case: Maintenance & Care: This vehicle has been maintained strictly according to Toyota’s recommended intervals and has been handled with the utmost care. There has been no external impact or misuse of the liftgate. Safety Risk: The inability to securely latch the trunk poses a severe safety hazard. I am concerned about the liftgate's stability at highway speeds and the potential for exhaust fumes to enter the cabin. Consequently, I no longer feel safe using this vehicle for my daily commute. Request for Action: Given that this appears to be a known issue within the 2019–2021 RAV4 production years, I am requesting that Toyota stand behind its reputation for quality and safety.     When trying to start my vehicle this morning, the car was having a noticeable issue the minute I turned on the vehicle. It was making some weird noises under the hood, and it was starting to lose power. Then the engine would cut out and a warning would appear on my dash that read, "Engine Stopped, Steering Power Low" Thank God this happened while I was at home. What if this had happened while I was driving down the road, at a red light. I would have been a sitting duck in the middle of the road and would not want to put my kids or I in jeopardy of being hit.

    I received recall notice December 2023 for defect in front passenger seat airbag that could cause the airbag to fail to deploy in the event of a crash. I have contacted the dealer several times and have been told each time that there is no remedy (yet). It has been two years and the risk to passengers in my vehicle has not been remedied. Fortunately, I have not had any collisions but the danger and safety concerns remain, and there has been NO ACTION BY TOYOTA ON THIS SERIOUS RECALL NOTICE The Recall Notice stills says “no remedy” and it has not been updated in the past year. This is a serious, potentially dangerous and life-threatening defect, a recall has been issued, and yet NOTHING has been done to remedy the defect FOR TWO YEARS. What remedies are available to me as a customer? Will action be taken on this complaint? Please let me know when I can expect a response.

    The door lock actuators required repair on all my doors for the RAV4 2020. They stopped working after the 3 year warranty was up. This is a safety issue. I had to manually lock and unlock my doors, which left in a dangerous position at night. Over the course of a year all 4 doors needed to have the actuators replaced. This happrened in 2024.

    The paint on my 2020 Toyota RAV4 (Blueprint color) is peeling and deteriorating prematurely, on the hood. The vehicle has been properly maintained and has not been in any accidents. The paint started peeling without any external cause. I’ve noticed similar complaints from other 2020 Toyota RAV4 owners, especially those with the Blueprint color, reporting the same issue. This suggests a widespread paint or clear coat defect from the manufacturer. The peeling paint not only affects appearance but also exposes the metal body to rust and corrosion, which could eventually compromise the vehicle’s safety and structure. Toyota has not issued a recall or offered repairs for this known problem, and many owners are being asked to pay out of pocket for costly repainting. I am requesting that NHTSA investigate this issue as a potential manufacturing defect and urge Toyota to take corrective action through a recall or warranty extension for affected vehicles.

    For the last 2 months, a code comes up it says loss of electric power steering assist. It came up 15 times in two months. What happens is a steering wheel ceases up for a second or two and then the code goes away. This is something that could happen anytime especially if I have my grandchildren with me and ex steering wheel could freeze up and I wouldn’t be able to steer around an accident at an intersection or something. This is very hazardous to me and my family. There was a recall on some of the 2020 Toyota RAV4’s in the past. I’m telling you I’m not a liar. I do get this coming up this way too many times.

    What component or system failed or malfunctioned, and is it available for inspection upon request? The flow shut-off valve (coolant leak) failed. This exact component was later included in Toyota’s official recall. The defective part was diagnosed by a mobile mechanic and replaced at a licensed independent auto repair shop. The failed component itself is no longer available, but all receipts and repair documentation are available upon request. How was your safety or the safety of others put at risk? The failure of the flow shut-off valve caused a coolant leak that created a significant safety hazard, as it affected the vehicle’s performance and drivability. This posed a risk to both my safety and the safety of others on the road, which is why I had the repair completed immediately before the recall was issued. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The issue was first diagnosed by a mobile mechanic and then confirmed by a reputable independent auto repair shop, who identified the flow shut-off valve coolant leak as the root cause. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No inspection was conducted by Toyota, police, or insurance representatives. However, Toyota later issued a recall for this same component, confirming it as a known safety defect. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Yes. The vehicle experienced noticeable performance issues prior to the failure, which prompted me to call a mechanic. I did have multiple warning message and coolant leaking in addition to the performance issues of the vehicle. These symptoms appeared several months before Toyota’s recall notice for the flow shut-off valve coolant leak arrived.

    We bought a 2020 Toyota Rav4 Limited with Sunroof. When we were driving in freeway, we noticed that a glass shattered somewhere in my car. We realized that the sunroof was completely shattered. There was no accident, just a heavy rain. We didn't know what to do, we didn't know if we have to call police in a matter as it was a potential safety hazard. We have seen a lot of complaint about this and there is no option to completely sealed it off or get rid of sunroof. We are now scared of driving this car in long distance, we will be trading this vehicle soon because of the incident.

    The contact owns a 2020 Toyota Rav4. The contact stated that while driving at an undisclosed speed, the message “Engine Maintenance Required” was displayed. The vehicle was taken to an independent mechanic, where it was diagnosed, and determined that the heater control valve had failed and needed to be replaced. The vehicle was repaired. The dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 47,000.

    Shorted Coolant Bypass Valve, only 14500 miles on my 2020 Rav4, Class action Suit and dealerships want you to pay $800-$1000 to fix problem with a leaky defective plastic poorly designed valve that eventually leaks into the electrical connectors and shorts circuits the valve. There is no recall for this fix and Toyota won't honor the repair. This is a common issue and Toyota Turns a Blind Eye. Error Message: Engine Maintenace Required Call Toyota Dealership. I Did and they give me a song and Dance stating there is no recall for this issue. If i run the heater in the car the message for a few a minutes, then shut off vehicle maintenance warning will reset, once I turn on the AC it will come back on. So, I guess I have to keep the heater on all summer.
